Q

What does the upright view best used to assess

What does a PA chest x-ray best used to assess?

A

Upright: better visualization of air-fluid levels (small bowel obstruction)

PA Chest x-ray: free air underneath hemidiaphragms & check chest pathology

Q

What are dark areas on x-ray considered? What is an example

What are white areas on x-ray considered? What is an example?

A

Dark = radiolucent, permits penetration of x-ray
Air

White= radioopaque, blocks penetration of x-rays

Q

What is drank on a UGI series?

What views are seen after drinking this?

A

Patient drinks barium sulfate contrast & fluoroscopy

Visualizes:

Q

What does a fluoscopy view considered? How does it work?

A

Fluoroscopy = “xray moview”

Continuous x-ray is passed through the body part, & the image is transferred to a monitor

Q

How is a Barium enema performed?

What body parts is it trying to view

16
Q

What diseases can be found with a Barium enema?

A

Inflammatory Bowel Disease
Cancer
Ulcers
Diverticula

17
Q

What does an apple core finding during barium enema indicate?

A

Adenocarcinoma

18
Q

What are contraindications of a Barium Enema?

A

Pregnancy (unless benefits outweigh risks)
Megacolon
Suspected perforation (use Gastrografin NOT barium)
Unstable vitals

19
Q

What contrast is ordered in suspect Perforation in a UGI series?

A

Use water-soluble Gastrografin

aka Leakage of barium outside of the UGI tract

20
Q

What kind of ultrasound is ordered to assess blood flow?

A

DOPPLER U/S

21
Q

What do dark areas vs light areas on Ultrasound?

A

Dark areas are echo free = hypoechoic
Eg. aorta, bile ducts, cysts

White areas are echogenic = solid
Eg. tumors

22
Q

How is a HIDA scan performed?

A

Patient receives radionuclide injection
Radionuclide extracted by liver & excreted in to bile
Provides visualization of biliary tree

23
Q

What is a suspected finding that would prompt you to order a HIDA scan?

A

Primary: indication is to diagnose suspected acalculous cholecystitis

24
Q

Which type of imaging is ordered first, U/S or HIDA?

A

US generally ordered 1st,

HIDA scan done if US negative & clinical suspicion for cholecystitis persists

25
What findings indicate cholecystitis during a HIDA with CCK and ejection fraction?
No radionuclide seen in GB within 15 – 60 min. Ejection fraction < 35% (< 50% is indeterminate) Test reproduces patient’s symptoms
26
What are contraindications to HIDA scan?
Pregnancy
27
What type of patient can a ultrasound be beneficial in that is contraindicated to other imaging?
Abd. ultrasound
28
Contrast is usually order to enhance imaging on CT of the abdomen, when is contrast contraindicated?
Renal Stones
29
What is the test of choice for supsected gallbladder stones?
Ultrasound --> stones are more easily detected?
30
What are indications to order an abdominal CT scan?
``` Liver diseases Pancreas GI - Tracto/ Bowel Spleen AAA ```
31
What are contraindications for for Abd. CT scan?
Pregnancy (unless benefits outweigh risks) Unstable vitals Morbid obesity (weight of patient on table)
32
What are contraindications for IV contrast?
Allergy to iodine or shelfish
33
What medication needs to stopped prior to CT with IV contrast?
Metformin
34
What is considered gold standard for abdominal concerns?
esophagogastroduodenoscopy EGD Can biopsy along with looking
35
What are contraindications towards using EGD?
Esophageal diverticula - ↑ risk of perforation Suspected perforation – can be worsened by insufflation of pressurized air Recent upper GI tract surgery – weak anastomosis site
36
What are therapeutical benefits of EGD?
Band esophageal varices Dilate esophagus Control bleeding
