What vessels supply the greater curvature of the stomach?
Left and right gastro epiploic arteries and veins coming off the splenic artery and vein
What arteries and veins supply the lesser curvature of the stomach?
Left and right gastric arteries (coeliac trunk and common hepatic respectively), left gastric vein off the portal vein
What is the nerve supply of the stomach?
Greater and lesser splanchnic coming off the sympathetic trunk, vagus, coeliac plexus, oesophageal plexus, peri arterial plexus
Lymphatics of the stomach?
Coeliac lymph nodes and left gastric lymph nodes
Blood supply of the oesophagus?
In thirds, inferior thyroid, branches off the aorta and left gastric arteries. Inferior thyroid veins, azygos system and portal system via left gastric vein
Lymph drainage of oesophagus?
Paraoesophageal plexus to posterior mediastinum, lower parts into left gastric and coeliac nodes
What are the 3 phases of swallowing?
Oral, pharyngeal and oesophageal
Complications of decreased acid in stomach?
Carcinoids as high gastrin levels cause overgrowth if endocrine cells
How many cm metaplasia does classical Barrets cause in the oesophagus?
3 cm
What is metaplasia?
Change from one tissue type to another
What are the treatment options for Barrets?
PPI, fundo plication, radio frequency ablation, laser therapy and photo dynamic therapy in the presence of intense acid suppression
Common oesophageal cancers?
Squamous cell and adenocarcinoma?
What is PEG?
Percutaneous endoscopic gastric feeding tube
What’s TNM staging?
Tumour (1-4), node (0-1), metastasis (0-1)
What protects the epithelial surface?
Prostaglandins, bicarbonate, mucus, blood flow, tight epithelial junctions
