GERD
A
- Heartburn lasting more than 3 months and/or refractory to OTC therapies (heartburn is always GERD)
- Dyspepsia (discomfort in epigastrium -pain, fullness, burning, bloating, early satiety) -not always GERD

Diagnosis
A
- based on symptoms
- then do trial of therapy
- endoscopy (barretts, esophagitis) - pt has pain or difficulty swallowing

Self care
A
- Antacids
- H2RAs
- PPIs
- occasional heartburn

GERD treatment
A
PPI for 8 weeks

Chronic GERD
A
- Symptoms come back then titrate PPI to the lowest effective dose and use prn dosage
- limits AE by titrating
- Can use prn antacids or H2RAs for episodic relief

A
thrombocytopenia, inc infection risk, inc osteoporosis risk, inc AKI and CKD risk, inc CNS effects
