General treatment strategies Flashcards
Bone fractures
hematoma osteoblasts and fibroblasts soft callous hard callous final ossification
Edema
PRICE
Optimal loading
stimulate lymphatic drainage
Articular cartilage injury
Low load to stimulate activity
DO NOT immobilize
E stim
Contractures
Reabsorb short fibers and replace with long fibers
replace with ordered fibers
after 14 weeks, might have to consider tearing
Connective tissue injury
Wolff’s law
realign fibers, strengthen, promote healing with appropriate stress
Muscle
movement and contraction to stimulate activity, synovial circulation
optimal loading
Chronic wounds
Maintain moist environment - not too dry, not too wet
Promote blood flow to the area (via e-stim, ultrasound)
Prevent against infection
Acute wounds
Remove infection via debridement, antimicrobials
Maintain moist environment
Remove/manage exudate/discharge from infection
Promote blood flow via e-stim
acute tendonitis vs tendonosis
Acute tendonitis - acute inflammation management; optimal healing; prevent further injury, and then start to gradually load it
Chronic tendonosis - Need to kickstart healing process.. May start with soft tissue techniques, something to instigate low level inflammation to restart healing. Also, eccentric, higher load on tissue, to get tissue to remodel.
hoffa massage
increased blood flow, relaxing
friction massage
avoid in acute injuries
analgesic effect
myofascial release
fibromyalgia, postural deviations
trigger point release
ishemic pressure, taut muscle or fascia
