Orthopnoea, paroxysmal nocturnal dyspnoea

L: Bi-basal crackles, third heart sound (S3)

R: Peripheral oedema, raised JVP

Ejection systolic murmur radiating to neck, narrow pulse pressure

When to suspect recurrent small PE as a cause for SOB?

A

Predisposing factors e.g. malignancy

Pleuritic chest pain, haemoptysis, vague symptoms

Tachycardia and tachypnoea often acutely

Right heart failure in severe cases

A

Obesity hypoventilation syndrome

Leads to resp acidosis and type 2 resp failure
