What are the six rights of medication administration?
Time, patient, route, dose, drug, documentation (TPRDDD)
When are the three medication checks of the six rights of medication administration?
in the medication room when you pull the med from the cabinet; in the medication room after medication prep complete (and after barcode scan); in the patient room at the patient bedside before administering the medication
When do multidose vials expire (medication)?
28 days after opening
When do vaccines expire?
When the manufacture says it does
When do you ask about allergies?
Before preparing the medication; at the bedside before giving the med
When should you document giving a med?
Right after you administer
Explain how barcode scanning works when administering med
Pull up patient chart on computer in med room; complete first check of the six rights; scan the barcode on med vial; med info should automatically populate unless there’s an error message; complete second check of six rights; complete third check of six rights
What meds are MAs not allowed to give
insulin; chemo; sedatives; IV emergency meds; blood and blood products; controlled substances; antianginal meds; investigational meds; anti-arrhythmic meds
What are some adverse reaction symptoms
vasovagal syncope; nausea; vomiting; redness, swelling, continued pain, numbness, tingling at injection site
What are some signs of anaphylactic reaction
rash; hives; itching; facial swelling; chest pain; difficulty breathing
What do you visually inspect med and label for
no particulates; no loss of integrity; not expired; no discoloration
What do you label your med with
med name; med strength; med amount; diluent name and volume (if used)
What are the steps when using liquid med
use medicine cup/syringe; shake all liquid before dispensing; put container lid inside up on counter; for med cup, measure liquid at eye level; stay until med taken completely
what is the post-injection process
engage needle safety shield and discard sharp; visualize opening of sharps container before discarding to prevent getting stuck; change sharps container when 3/4 full; never recap needle after giving injection
What to do when you do med error
notify provider; provider should notify patient of error; document only what you gave in epic; do RL; don’t put ll details from RL or that you did RL in epic
