Characteristics of patients under anesthesia

A

-Analgesia (an absence of pain)
- A state in which amnesia occurs

Q

Theories to the mechanism of anesthetic agents

A

-Protein receptor- proposes general anesthetic affect specific proteins in the cns
- Meyer-Overton= based on the correlation bt solubility and anesthetic potency
- Endogenous endorphins (opiate like substances suppress pain

A

The pt has a mild systemic disease tht does not limit activity, such as BMI>30 but <40, controlled hypertension, asthma, bronchitis, diabetes, or smoking w/o copd

Q

What ways is anesthesia given

A

IV induction allows pt to receive
Inhaled anesthetic agents through a breathing circuit and anesthesia machine. Inhaled agents are controlled using vaporizer on the anesthesia machine. Unless there is a reason not to, anesthetic are administered primarily inhaled agents

Q

Barbiturates

A

Most commonly used is sodium Pentothal, sodium thiamyl, methohexital sodium. They are short acting. Does not provide analgesia

Q

Nonbarbiturate agents

A

Propofol used for induction and maintenance. It is lipid emulsion. It has short half life making it ideal for sedation with minimal side effects. It’s used as a preventative and rescue antiemetic.

17
Q

Étomidate

A

A nonbarbituate induction agent that has minimal effects on myocardial metabolism, cardiac output, and peripheral or pulmonary circulation. It is generally utilized for pts w/ cardiac history and pt’s who cannot tolerate dramatic changes in bp
