chronic thrombosing inflammatory disease of small and medium arteries and veins of arms and legs
epidemiology
uncommon in US
Much more common in Asia
male
Onset usually in 20s (before age 40)
Pathology
two phases
- acute phase
- chronic phase
acute phase
segmental transmural acute inflammation, with granulomas and giant cells
WITHOUT necrosis
most characteristic of acute phase…
thromboses with “microabscesses” in thrombus
chronic phase
nonspecific organization and recanalization of thrombus w/ neovascularization and fibrosis
Signs and symptoms
instep claudicaton
cold feet
Raynaud’s phenomenon
migrating superficial nodular thrombophlebitis
complications
ulcers and gangrene
Diagnosis
History and physical exam!
Angiogram?
Angiogram shows
stenosis (more distally) with corkscrew collaterals
Treatment
cessation of smoking
amutation

vasospasm of finger arteries w/ cold exposure
causes tri-color response:
- fingers turn white from cutoff of arterial blood supply
- then blue from accumulation of deoxygenated hemoglobin
- then red from resumption of arterial blood flow
What is the most essential aspect of treatment of Buerger disease?
getting the pt to quit smoking
Buerger disease is associated with
SMOKING!!! (100%)
