Members who witness or have a role in a near miss should report the incident either by contacting a BSO immediately or through the Near Miss Report located on the MFMD Safety InsideMesa page.
Fill out and submit the information, following the directions on the form.
BSO’s are responsible for conducting the discovery phase of near miss reviews. Members should expect to be interviewed and asked to recall as much about the incident as possible. Event identifiers will be scrubbed and every effort to maintain anonymity will be made.
Discovery information will be forwarded to the Safety Team for peer analysis. After peer analysis the division in which the incident took place will be charged with developing a communications plan for disseminating the findings to all members and making any alterations to training or procedures.

How does the STAR program work?

Any member of the department can request a STAR award be issued for any other member and no supervisor approval is required. Variables such as rank, civilian/sworn, or supervisor/subordinate are not a factor in utilizing the STAR Program.
To recognize a member, send an email to MFMD Personnel or directly to the STAR Rep. The email should include the employee’s name, employee number and what the person is being acknowledged for.

If involved in a moderate to severe accident assess the need for medical care call 911 and tell the 911 operator you are a COM Fire Department employee. The COM Alarm Room or closest BC will notify the Duty Chief of the incident. Law enforcement will investigate the accident and issue citations or make arrests. A BSO, once given permission by law enforcement, will guide the member through the accident reporting process, take photos and conduct the discovery phase of the accident.

If the accident is minor and minor property damage has occurred, call the MRDC Alarm Room Supervisor at 480-521-4275 and have them notify the closest MFMD Battalion Chief. A BSO will likely respond to your location and guide the member through the accident reporting process, take photos and conduct the discovery phase of the accident. Reports should be forwarded through appropriate channels within 24 hours.
