Accountability for system failures. Action plans that come about as part of necessary and regular program evaluation could result in a number of realizations and outcomes. It could be decided, for instance, that some services that have been provided are not productive of results, or that more resources should be spent on supporting productive services. Other ideas and action items are possible depending on the situation and type of analysis.
Accountability for failures is not usually considered part of a forward-looking plan for improvement. Issues of accountability may exist and may come to light during a program evaluation, but program improvements and plans of action or correction should not be phrased as failures of the program itself.

Operant Conditioning
Pioneered by BF Skinner. There are four kinds of responses to behavior that will shape that behavior. All reinforcement increases behavior, and all punishment decreases behavior. A negative reinforcement would be taking away a negative stimulus, while a positive reinforcement would add a positive stimulus. A positive punishment would add a negative stimulus, and a negative punishment would take away a positive stimulus.
