What is a hiatal hernia?
Herniation of a portion of the stomach upward into the chest through a di- aphragmatic opening
What are the two types of hiatal hernia?
sliding and paraesophageal
Characteristics of sliding hiatal hernia?
Sliding hiatal hernias (95%): The gastroesophageal junction and a
portion of the stomach are displaced above the diaphragm.
Characteristics of paraesophageal hiatal hernia?
Paraesophageal hiatal hernias (5%): The gastroesophageal junction remains below the diaphragm, while a neighboring portion of the fundus herniates into the mediastinum.
Treatment of sliding hiatal hernia?
Medical therapy and lifestyle modifications to ↓
GERD symptoms.
Treatment of paraesophageal hiatal hernia?
Surgical gastropexy (attachment of the stom- ach to the rectus sheath and closure of the hiatus) is recommended to prevent gastric volvulus.
