Q

VAAs effects on ventilation

A

depresses ventilation mediated directly through medullary centers and indirectly through intercostal muscle function, RR tends to increase, TV tends to decrease …. drive to breath from hypoxia abolished at 1 MAC

Q

VAAs effect on airway resistance

A
  • decrease airway resistance by relaxing bronchial smooth muscle
    NOTE: VAAs are potent vasodilators

20
Q

VAAs mechanism of action

A
  1. stimulate inhibitory receptors
    • in the brain… VAAs increase amount of time GABA-A channels are open
  2. Inhibit stimulatory receptors
    - in the spinal cord… will produce immobility in ventral horn. site of action= glycine (stimulation), NMDA- glutamate (inhibition) and Na channel (inhibition)
21
Q

Which VAA worsens bronchospam? whats the best choice for bronchospasm risk?

A

Des, Sevo

22
Q

When should you avoid N2O?

A

inner ear, intraocular, pneumo… avoid in pregnancy and B12 deficiency

NOTE: does add analgesic properties

23
Q

Why should you not overpressurize Des?

A

SNS stimulation
