Q

What’s the difference between an Anesthesia gas machine & an ICU ventilator?

A
1.Manipulation of mix of gases
    •Washin/out N2, O2, N2O, agents
    •Humidification
    •Rebreathing and CO2 removal
2.Monitoring of gas composition (because of #1)
3.Manipulation of fresh gas flow
   •“low flow” anesthesia
4.Adaptability
   •smallest neonate to largest adult

Q

What is the pipeline gas source?

A

It is liquid O2 converted to gas and supplied to hospital pipelines at 50 psi

Q

What is the DISS?

A

Diameter index safety system- prevents cross connection of gases by designating different size connections for each gas

16
Q

What is present at the pipeline inlet?

A
  1. Filter
  2. Check valve
  3. pressure gauge
17
Q

What does the check valve do?

A

ensures unidirectional forward flow so that a machine running on cylinder supplies with the hose disconnected a the wall outlet does not leak

18
Q

What are some pipeline source hazards?

A
  1. Unexpected contents- bacteria, viral, particulate
  2. Inadequate or (excessive) pressure
  3. Unexpected contents
19
Q

What would alert you to inadequate pressure?

A
  • Flowmeters drop, bellows drop, low pressure alarms

- To correct: disconnect hose from wall and open O2 cylinder fully, Don’t use mechanical vent and bag manually

20
Q

What might indicate unexpected crossover of gases?

A
  • Decreasing FiO2 in spite of unchanged O2 flow

- Disconnect hose from wall and open cylinder fully

21
Q

When should cylinders be opened?

A

Only when being checked or pipeline supply is not available

22
Q

What is the PISS?

A

Pin index safety system- the holes mate with pins in the yoke which point to where the cylinders are attached. It is a means of preventing misconnections

23
Q

How can the PISS be defeated?

A

if pins are missing, removed, or if more than one washer is used

24
Q

What is the hanger yokes function?

A
  1. orients cylinders
  2. provided unidirectional flow
  3. ensures gastight seal
25
Q

What size are the O2 cylinders on the back of the gas machine?

A

Size E

26
Q

What are check valve (or unidirectional valves) within the hanger yoke functions?

A
  1. Minimize transfilling and leaks to atmosphere

2. allows cylinder to be changed during case

27
Q

What is the job of the cylinder pressure regulator?

A

Converts the high variable pressure in the tank (up to 2000psi) to constant pressure of 45 psi downstream from regulator

28
Q

Who regulates cylinder transport and storage?

A

DOT (US dept. of Transportation)
