Define Gangrene
Death of tissue due to poor vascular supply.
What are the different types of Gangrene?
Dry: tissue death only
Wet: tissue death + infection
Pregangrene: tissue on the brink of gangrene
How does Gangrene usually present?
Black tissues
Slough
May be suppuration (pus forms)/sepsis
What micro-organism causes Gas gangrene?
Clostridium perfringes
Define Gas gangrene
Clostridium perfringes myositis (inflammation/degeneration of muscle tissue)
What are some of the risk factors for Gas gangrene?
DM
Trauma
Malignancy
How does Gas gangrene usually present?
Toxaemia Haemolytic jaundice Oedema Crepitus from surgical emphysema Bubbly brown pus
What is the management of Gas gangrene?
Debridement (may need amputation)
Benzylpenicillin + metronidazole
Hyperbaric O2
What is Synergistic gangrene?
Mix of aerobes + anaerobes. Can progress rapidly to necrotising fasciitis + myositis
What is the management of Synergistic gangrene?
Take cultures
Debridement (including amputation)
Benzylpenicillin ± clindamycin
