Spastic gait is a manifestation of
Stroke or Demyelination, Cerebral Palsy
Paraplegia if scissoring
Hemiplegia
If the gait is spastic you would examine for
upper motor neuron signs
sensory level if paraplegia
Cerebellar gait is a manifestation of
Alcohol or cerebellar degeneration
If the gait is cerebellar examine
for other cerebellar signs
Parkinsonian
Parkinson’s disease: lack of arm swing, stooped posture
If gait is parkinsonian
Examine for bradykinesia, hypomimia, rigidity, assymetric tremor
Exclude DDx incontinence, frontal release signs
alien limb sign, PSP, cerebellar signs
Marche à petits pas
Preserved arm swing and upright posture
Diffuse small vessel disease
Sensory ataxic gait
Stamping gait, wide based and apprehensive
Friedrich’s Ataxia, Subacute combined degeneration, Tabes Dorsalis, Cervical myelopathy
If the gait is a sensory ataxic gait
Examine for loss of vibration and proprioception sense
