Who usually get Cryptococosis meningitis
A
- ICPs like HIV infected pts

How does one acquire cryptococcos?
A
inhalation of yeast-like cells from soil contaminated w/ pigeon droppings
- not dimorphic, always an encapsulated yeast

How can one Dx cryptococcosis?
A
- CSF of meningitis pts - treat w/ India ink, look for halos
- CSF testing for antigens

What is the treatment for cryptococcosis?
A
- amphotericin B and flucytosine combo therapy fro CNS cases

What is the causative organism of Steroid-associated meningitis?
A
- Exserohilum rostratum- always a mold
diagnosed by CSF culture and PCR

What is the treatment of steroid-associated meningitis?
A
- IV voriconazole for 3 months or more
- severe cases should get Amphotericin B
- injection recipients who are Asx should be observed
