What are employer’s responsibilities?
- must need signed consent (Secrion B) for release of functional abilities info
- where available, attach list of worker’s job activities to assist in completing info
Who pays the health professional to fill out form WSIB generic form only?
WSIB (then the WSIB charges the employer)
What can the employer use instead of the generic WSIB form?
an employer specific workplace form that provides consent of worker’s realease of information
When does the employer pay the health professional directly?
If employer uses own generic form for hralth professional to fill out

What are the Health Professionals responsibilities? (5)
- completion of FAF in full (does not require specialized functional abilities evaluation)
- do not include diagnostic or confidential information
- add sepcific info on duration of temp. restrictions/ max. times or weights to be considered
- once complete, promptly and give to worker and employer
- fax or mail to WSIB for payment
