The amount of time that has passed since the pain or symptom began

The frequency of the pain or symptoms

The type of pain
Severity
The intensity of the pain
Modifying factors
What makes the pain better or worse
Associated symptoms
Symptoms related to the chief complaint
EKG
A test that checks for problems with the electrical activity of the heart
Family history
Contains the hereditary conditions of the patients family members
