Risk factors for child abuse
Low socioeconomic status, Unplanned pregnancy, Twins, Pre-term children, Special needs children
most common injury in child abuse
skin lesions- burns, bruises, lacerations, bites
Fractures that are highly specific for abuse
- posterior rib fx.
- metaphysical corner fx.
- Complex skull fx.
- Long bone fx before walking
- b/l fx
- fx in various stages of healing
- scapular fx
- outer end clavicle fx
- vertebral fx/subluxation
- digital fx in non-mobile
most common long bone fxs in abuse
humerus> tibia > femur
the most pathognomonic fracture of abuse in children
a transverse fracture- direct trauma to bone- being struck by object
metaphysial corner fractures
bucket handle fx. caused by violent shaking, forceful pulling. LE>UE
Spiral fractures
are not correlated with abuse
indications for skeletal survey
mandatory in any child
Skeletal survey components
- Ap and lateral skull, chest and extremity of interest
- right and left oblique of chest
- AP and lateral pelvis, long bones and feet
- PA oblique of hands and feet
Rubin criteria for neuroimaging
if any apply, obtain neuroimaging:
1. 1 fx
- facial bruising/injury
follow up for fx of abuse
repeat XR in 2 weeks for dating of injury
