Aspirin adult dose
Chest pain ACS, Chest pain, STEMI
324mg PO (81mg tablets x4)
Adenosine adult dose
Tachycardia stable, Tachycardia unstable
6-12mg rapid IV/IO push
Albuterol adult dose
Allergic reaction, Drowning, Hyperkalemia, Respiratory distress
2.5mg in 3.0ml SVN; repeat until improvement
Amiodarone adult dose
Cardiac arrest, Tachycardia stable, Tachycardia unstable
300mg IV/IO; may repeat one dose of 150mg if refractory after 5th shock
Atropine adult dose
Bradycardia, Organophosphate poisoning
Bradycardia: 0.5mg IVP; may repeat q 3-5min
Organophosphate poisoning: 2.0mg IV/IO q 15 min as needed to decrease secretions and ventilatory resistance.
Calcium chloride adult dose
Bradycardia, Hyperkalemia, Overdose/Poisoning
1.0gm (10% solution) slow IV/IO push
Diazepam adult dose
Behavioral emergency, Obstetrical emergency, Seizure, Ventilation management, Electrical therapy/Transcutaneous pacing
5.0mg IV/IM/IO may repeat titrating to effect
Benadryl adult dose
Allergic reaction
50mg IM/IV/IO/PO
Dopamine adult dose
Bradycardia, Pulmonary edema/CHF, Target temperature management &post resuscitation
5-20mcg/kg/min; Titrate to SBP >90mmHg
Droperidol adult dose
Abdominal/flank pain nausea vomiting, Behavioral emergency, Pain management
1.25mg IM/IV/IO followed by a saline flush or bolus; may repeat the dose after 5 minutes
Epinephrine adult dose
Allergic reaction, Cardiac arrest
Allergic reaction: 1:1000, 0.5mg IM; may repeat q 15 min up to 1.5mg max
Push dose epi: 1:100000 5.0mcg-10.0mcg IV/IO may repeat q 2-5min to maintain SBP>90 ( 1mL epi 1:10000 + 9mL normal saline = 10mcg/mL)
Cardiac arrest: 1.0mg IV/IO q 3-5min (ETT administration requires 2-2.5 times the dose)
Etomidate adult dose
(Tachycardia stable, Tachycardia unstable, Ventilation management, Electrical therapy/Synchronized cardioversion, Electrical therapy/Transcutaneous pacing, Endotracheal intubation)
Induction: 0.3mg/kg IV/IO
Sedation: 0.15mg/kg IV/IO
Fentanyl adult dose
Pain management, Electrical therapy/Synchronized cardioversion, Electrical therapy/Transcutaneous pacing
Up to 1.0mcg/kg IN/IM/IV/IO to a max single dose of 100mcg. May repeat dose after 10 minutes. Additional doses require physicians order
Glucagon adult dose
Altered mental status/Syncope, Bradycardia, Overdose/Poisoning, Seizure
Low BGL: 1.0mg IM for no IV access
Bradycardia: 1mg IV/IO for patients on beta blockers
Beta blocker OD: 1.0mg IM/IV/IO may repeat once in 3-5 min
Glucose adult dose
Altered mental status/Syncope, Seizure
Oral: 15gm glucose between the gum and cheek if gag reflex is present
D10 (10% dextrose in 250mL NS): 25gm IV/IO may repeat x1 in 5 minutes
