DD Form 254– DoD Contract Security Classification Specifications
This form provides to the contractor (or a subcontractor) the security
requirements and the classification guidance that would be necessary to
perform on a classified contract.
DD Form 441– Department of Defense Security Agreement
A contractual agreement between the US Government and a cleared
contractor facility.

Agreement
This is an attachment to the DD Form 441, Department of Defense Security
Agreement, that lists cleared divisions or branch offices that are included in
and covered by the provisions of the organization’s Security Agreement and
Certificate Pertaining to Foreign Interest.
DD Form 1540– Registration for Scientific and Technical Information Specifications
Much of the data that the Defense Technical Information Center (DTIC)
handles is controlled, either because of Security Classification, or Distribution
Limitations. All potential users are required to register to obtain DTIC
services. The registration procedure is mandated in DOD Instruction (DODI)
3200.14, Principles and Operational Parameters of the DOD Scientific and
Technical Information Program (STIP). Federal Government Agencies and
their contractors are required to complete the DD Form 1540. The contractor
community completes a separate DD Form 1540 for each contract or grant,
and their registration is valid until the contract expires.
DD Form 1847– Sensitive Compartmented Information Indoctrination Memorandum
Used to precisely indentify individuals when it is necessary to certify their
access to sensitive compartmented information.
DD Form 1847-1– Sensitive Compartmented Information Nondisclosure Agreement
Used to precisely indentify individuals when it is necessary to certify their
access to sensitive compartmented information.
DD Form 1848– Sensitive Compartmented Information Debriefing Memorandum
A memorandum records the fact that an individual was debriefed on a
Sensitive Compartmented Information (SCI) Special Access Program(s)
DD Form 1879
Request for Personnel Security Investigation. Used to request an SSBI, PR,
SII, or ENAC.
DD Form 2024– DoD Security Classification Guide Data Elements
This form is executed by the originator of each security classification guide
issued pursuant to the requirements of DoD Regulation 5200.1-R,
“Information Security Program Regulation,” to report:
Approval (promulgation) of a new security
classification guide
Revision (change) of a security classification
guide
Reissuance (reprinting to incorporate previous
and/or current revisions or changes) of a classification guide
Accomplishment of the biennial review of a
classification guide
Cancellation of a classification guide, and
Correction of data furnished on previously submitted forms.
DD Form 2056
Telephone Monitoring Notification Decal. Used to notify a user that the
telephone is subject to monitoring at all times. Use of that telephone
constitutes consent to monitoring.
DD Form 2501–

Authorization
Used to identify appropriately cleared DoD military and civilian personnel
who have been approved to hand carry classified material in accordance with
the following:
(1) The individual has a recurrent need to hand carry classified
information;
(2) The form is signed by an appropriate official in the individual’s
servicing security office;
(3) Stocks of the form are controlled to preclude unauthorized use.
(4) The form is issued for no more than one year at a time. The
requirement for authorization to hand carry shall be reevaluated and/or
revalidated on at least an annual basis, and a new form issued, if
appropriate.
(5) The use of the DD Form 2501 for identification/verification of
authorization to hand carry Sensitive Compartmented Information or
special access program information shall be in accordance with
policies and procedures established by the official having security
responsibility for such information or programs.
DIS FL 381-R– Letter of Notification of Facility Security Clearance
Document notifying a facility they it is cleared to handle classified material.
This document must be surrendered when the clearance is revoked or
suspended.

Additional PSQ and NAQ forms may be ordered from the Defense Industrial
Security Clearance Office (DISCO) using this form.

Consent
The form used by DISCO to notify a contractor that a PCL or a Limited
Access Authorization has been granted to an employee.

SF 85– Questionnaire for Non- Sensitive Positions
A permanent document that may be used as the basis for future investigations,
security clearance determinations, and determinations of suitability for
employment. Responses to this form may be compared with previous
security questionnaires.
SF 85P-S– Supplemental Questionnaire for Selected Positions
A permanent document that may be used as the basis for future investigations,
security clearance determinations, and determinations of suitability for
employment. Responses to this form may be compared with previous
security questionnaires.
SF 86– Questionnaire for National Security Positions
A permanent document that may be used as the basis for future investigations,
security clearance determinations, and determinations of suitability for
employment. Responses to this form may be compared with previous
security questionnaires.

Form used to track the disposition of classified material (transfer, inventory,
destruction, hand receipt, or other).
SF 311 Agency

Program Data
The SF-311 is a data collection form that every Executive Branch agency
submits on an annual basis to report the total number of original classification
authorities, classification decisions, mandatory review requests, and
declassification decisions for that particular agency. The data collected from
these forms are reported in the Annual Report to the President.
SF 312 (SF 189, SF 189A)
Classified Information Nondisclosure Agreement. A contractual agreement
between the U.S. government and a cleared employee, in which the employee
agrees never to disclose classified information to an unauthorized person.
SF 328 – Certificate Pertaining to Foreign Interests
The NISPOM requires that a SF 328 be submitted during the initial facility
clearance process and when significant changes occur to information
previously forwarded.
SF 700 –

Information
SF 700 is a two-part form consisting of an envelope with a tear-off tab and
cover sheet. The cover sheet and face of the envelope provide space for
information about the activity, container, type of lock, and who to contact if
the container is left open.
