Forceps, elevators and Luxators Flashcards
What happens if the bone is stronger tan the tooth?
It will break at bone level (subgingival) - requires surgical removal
How to remove a tooth that is weaker than the bone?
Surgical removal
Drill away bone around the tooth to effectively expand the socket
Upper of lower?
Upper
Upper of lower?
Lower
Where do you use the forceps on the tooth?
Root of tooth at bone level
Who is more likely to have root caries?
Old people
Perio pt
Why not grab the crown?
Usually has caries
Why do you initially push down th elong axis of the tooth?
Compress tooth - e.g. tiny micro cracks gets compressed
Strongest state
Is the buccal side of the maxillary arch string of weak?
Weak - move tooth this way
What movement do you not do with multirooted teeth?
Rotataion - root fracture!
Steps in extraction?
Straight line axis for upper anterior?
Excellent straight line axis
Nice straight forecep
Where to place fingers for upper anterior extraction?
Buccal and palatally
Use for what?
Upper anterior - straight forceps
Use for what?
Upper premolar forceps
Why be careful rotataing an upper 4?
Pull buccally - rotation may leave root apices behind
especially on orthodontic pt - root fragment left behind can displace 5 as gap is closed from braces
Used for what?
Upper molar
Used for what?
Support TMJ
Beak to cheek ?
Yes
Where to stand for bottom right ant?
Stand behind pt
Where to stand for lower left ant?
In front of pt
Forceps for lower molars?
Dual beak
Where to stand for lower right molar extraction?
Behind pt
What motion to expand socket for lower molar?
Figure of 8
Where to stand for lower left molar?
In front of pt
How to looses socket for lower Olaf’s?
Buccal-lingually
Then figure of 8 - make sire not to hot adjacent teeth
For lower molars, how is the pt sitting n chair?
Upright and Lower down
What to do after extraction?
Buccal plate squeezed back in
Achieved hemostasis quickly
What to use if there is no crown?
Luxators and elevators
What is more delicate - Luxators or elevator?
Luxators - use first
How to use elevator?
Used within apical space and rotational movement of the PDL space
Rotate
Go in buccal - move elevator palatally
= tooth move palatally and bone move bucally
Why keep the index finger extended toward the tip of a Luxators/elevator?
Preventing the instrument moving too far if it slips out of the PDL space and limiting damage to adjacent structures
How to use luxator?
Don’t twist
Small wiggle as you separate the tooth and PDL
Why not rotate luxators?
Fragile
Slip
Where in th emuthdo you not se luxators?
Lingually - risk of slipping and penetrating FOM
Palatally - slip and damage greater palatine/incisive nvb
Why not lever teeth out?
Crushed interdental bone / damage PDL
Mobility / avulsion of adjacent teeth
Sensitivity of adjacent teeth
Fractured restoration in adjacent teeth
Fractured basal bone
