Foley Validation Flashcards
Step 1:
- Perform Hand Hygiene before patient contact
- Verify correct patient using two patient identifiers. Utilize AIDET
Step 2:
Assess for:
- Allergies, specifically latex or povidone-iodine
- Patient’s readiness for procedure
Step 3:
Perform hand hygiene, clean gloves, assess and clean and assess perineal area
-Remove gloves and perform hand hygiene
Step 4:
Ask for mobility of hips and any past hip surgeries
-Position patient for procedure in a dorsal recumbent position by relaxing thighs
Step 5:
Open up kit
- Check for expiration date and any tears
- Use sterile technique and open the sterile inner package containing the catheter supplies
Step 6:
Place waterproof sterile drape (SHINY SIDE DOWN)
Step 7:
DON STERILE GLOVES (OUTSIDE STERILE FIELD)
Step 8:
Prepare sterile tray on bedside table before transferring b/w the pt’s legs to prevent contamination. SETUP EVERYTHING (TIP IN LUBRICANT, SALINE PREPARED)
(REMOVE CATHETER WRAPPING WILL BE REMOVED BEFORE TRANSFER)
Step 9:
CLEAN the URETHRAL MEATUS
- Clean labia majoris and minoris w/ antiseptic soaked cotton balls with forceps
- Away, Towards, Middle
- New cotton ball for each swipe
AROUND STERILE FIELD!!! NOT ACROSS
Step 10:
INSERT CATHETER
- Ask patient to bear down gently
- Advance catheter 2-3 inches until urine flows (STATE URINE FLOW)
- after urine flow, advance another 1-2 inches
Step 11:
INFLATE BALLOON
- After inflation, gently pull back until meeting resistance (DO NOT PUSH BACK IN)
- Place drainage bag below the bladder on bed rail
DON NEW GLOVES
-Secure Foley to pt and clip the gown
Step 12:
REMOVAL
- Hand hygiene/AIDET
- Raise Bed to appropriate height
- REMOVE ANCHORING DEVICE
- INSERT A STERILE SYRINGE AND WITHDRAW ALL STERILE SALINE; STATE mL WITHDRAWN
- Slowly pull out catheter wrapping around fingers
- Assess for COMPLETE REMOVAL of catheter
- Unhook catheter
- Ask for any pain or discomfort; thank pt
