OT is only setting up patient for task, e.g., cutting up food for eating.
QRP Level 4…
Supervision, Verbal cues, Touching/CGA, NO physical assistance
Assistance level: AD, Supervision, Verbal cues, Incidental touching
Physical assistance level: No physical assistance
*Physical assistance=helping perform an actual part of the activity.
*You do not have to click all 3, Sup/VCs/Incidental touching to get a score of 4
QRP Level 3…
Partial/Mod A
Assistance needed: Physical assistance
Physical assistance needed: Less than 25%
*Pt is performing >50% of work/therapist is providing <25%
E.g., Guiding hips into chair, helping pt to weight shift
QRP Level 2…
Substantial/Max A
Assistance needed: Physical assistance
Physical assistance level: 50-74% OR 75% or more
*Pt is performing <50% of work/OT providing >50%
OR
*Pt is performing <25% of work/OT is providing >75%
Even if pt is only helping 2% by lifting a foot for donning socks, or reaching toward for to attempt, it counts as effort & max a.
QRP Level 1…
Total Assist
Assistance needed: Physical assistance
Physical assistance level: Total assist
HR norms & abnormalities
Norm: 60-110 BPM
Abnormal: Above 130 & below 60
*Pts taking cardiac meds, such as Beta Blockers, may present with HR <60. Check chart & with RN prior to tx.
SPO2 norms & precautions
Norm 95-100%
Abnormal: Below or at 90%
If pt experiencing abnormal levels, change position to allow for recovery, alert RN.
Educate on breathing exercises; pursed lip breathing.
*O2 may be taken off if saturations on 2L or lower are within parameters set by MD. Check with RN.
*EXCEPTION: Chronic lung/cardiac conditions; COPD, emphysema, lung transplants, etc. as they have difficulty maintaining SPO2 higher than 90%.
–Check MD parameters in chart.
BP norms & precautions
Norm: 120/80
Abnormal: >180/110 & <90/60.
If abnormal BP, change pt’s position, check again, then alert RN.
*EXCEPTION: Pts may be taking meds keeping BP high or low. Check chart.
