Abdominal shape
Appearance of stool, emesis & urine: may need to ask patient if they have any.

Right 2nd intercostal space, parasternal.
what should you ask during a neuro assessment?
if you have numbness or tingling

Breath sounds 5 sites A & P
Breath sounds 2 sites lateral

Left 4th intercostal space, parasternal.

Orientation: Person, place, time, situation
