Lisinopril
Initial: 10 mg PO qd for HTN
Carvedilol
Initial: 6.25 mg PO BID for 2 weeks for HTN
Metoprolol
Initial: 25 to 100 mg PO qd for HTN
HCTZ

Amlodipine
Initial: 5 mg PO qd for HTN
Amoxicillin
ENT (mild-mod)/Skin/GU infection: 500 mg PO q 12 hrs or 250 mg PO q 8 hrs
Augmentin
Acute otitis media, mod: 500 mg PO q 12 hrs
Community Acquired PNA: 2000 mg PO q 12 hrs x 7-10 days
Skin infection: 500 mg PO q 12 hrs
Sinusitis (initial empiric tx): 500 mg/125 mg PO 3x/day for 5-7 days
UTI: 500 mg PO q 12 hrs
Bactrim
sulfamethoxazole 800 mg/TMP 160 mg PO q 12 hrs
doxycycline
100 mg PO q 12 hrs
azithromycin
500 mg PO loading dose (day 1), then 250 mg PO qd for days 2-5; can also do onetime 1 gm PO dose or 1 gm PO dose weekly
ciprofloxacin
500 mg PO q 12 hrs
Glyburide
Glycemic control: Initial: 2.5 to 5 mg PO qd, titrate up no more than 2.5 mg weekly as needed
Metformin
Immediate-release, initial: 500 mg PO BID
Extended-release, initial: 500 mg PO qd
Lantus (insulin glargine)
Insulin-naive: 10 units (0.2 U/kg) subcut. daily, adjust dose no more than every 3-4 days
Lispro Insulin
administer 15 min before mealtime, individualize per pt
