3rd generation fluroquinolones (gram + cocci)
Levoflxacin, moxifloxacin, gemifloxacin
*streptococci, MRSA
Pharmacokinetics
Oral
Antacid and food may interfere
Probenecid may block excretion.
Reduce dose in renal dysfunction.
Moxifloxacin: hepatic and biliary metabolism
Dont give in UTI
MOA
Inhibits topoisomerase 2+4
Block relaxation of DNA
POST ANTIBIOTIC EFFECT
BACTERICIDAL
Toxicity
GIT distress
Rash, headache, dizziness,insomnia
Abnormal LFTs, tendinitis, tendon rupture
Dont give to children and pregnant
Increases level of theophylline and methyxanthines
Prolong QT
Dont give wd anti arrhythmia
