Pharmacist license expiration
All pharmacist licenses expire on the same day and time biennially regardless of when issued; every two years on the odd years
*Next expiration date is September
30, 2025
Late license renewal
If licenses are not renewed by midnight on the expiration date the license is considered delinquent and the fee increases (64B16-26.1021)
If 120 days pass, the license is considered notified delinquent & the fee increases again
License will become null and void if not renewed within two renewal cycles and require entire new applicant license process to obtain a license
Do interns need to pay the Immunizing Certification Fee for Pharmacists?
No, interns do not have to pay this fee to be certified as interns
Consultant Pharmacist License requirements
. Licensed pharmacist in good standing
. Fill out application and pay fee
. Complete consultant pharmacy course of no fewer than 20 hours approved by FBOP Tripartate CE Committee
. Pass test at the end of course
Consultant Pharmacist License Renewal requirements
. 24 hours of CE biennially, in addition to the 30 hours of CE required for general pharmacist license renewal
. If renewed 12 months or more after initial licensure, 12 hours of CE required
. Less than 12 months, no CE required
Nuclear Pharmacist License requirements
. $55 license fee, $105 renewal fee
. Certificate of training from an approved program
- Min. 200 clock hours of didactic training
. Min. 500 hours supervised training within 7 years
- If training was not completed within 7 years, the
applicant must have been engaged in the lawful
practice of nuclear pharmacy in another jurisdiction
for at least 1080 hours during the last 7 years
