Symptoms:
● The hoist cable, hook, or rescue device becomes fouled or secured on the
deck or ground
● Hoist cable is visibly damaged (e.g., kinked, bird-caged, or broken strands)
HOIST CABLE FOULED/DAMAGED
HOIST CABLE FOULED/DAMAGED
If injury or damage is imminent:
- ACTIVATE HOIST SHEAR SWITCH WHILE ANNOUNCING - “SHEAR, SHEAR, SHEAR” (PF/PM/FM)
- ANNOUNCE - “CABLE IS AWAY”
If conditions permit:
- PAY OUT SLACK IN CABLE
- ANNOUNCE – “CABLE FOULED”
- DIRECT THE VESSEL CREW TO FREE THE CABLE
If unable to free the cable or the cable is damaged:
- CABLE - CUT WITH CABLE CUTTERS
- ANNOUNCE - “CABLE IS AWAY”
If mission dictates:
8. QUICK SPLICE - ASSEMBLE
Symptom:
● Hoist fails to respond to flight mechanic inputs
HOIST FAILURE
HOIST FAILURE
- ANNOUNCE - “HOIST FAILURE”
- ADVISE THE PF - “COMMITTED” OR “NOT COMMITTED”
If committed:
- CABLE AND DRUM - CHECK FOR FOULING
- EMERGENCY HOIST POWER - ACTIVATE
If hoist power is not restored:
5. MANUAL OVERRIDE - ACTIVATE
If hoist fails to respond:
6. RESCUE DEVICE - RETRIEVE
If unable to retrieve rescue device:
- CABLE - CUT OR SHEAR
- ANNOUNCE - “CABLE IS AWAY”
If not committed:
9. ABORT THE HOIST
Symptom:
● Hoist boom does not respond to electrical control inputs

HOIST BOOM FAILURE
- ANNOUNCE - “BOOM FAILURE”
- HOIST MASTER SWITCH - CHECK CREW POSITION
- HOIST ACT CONTROL AND HOIST ACT POWER CIRCUIT BREAKERS (2) - CYCLE (Hoist Boom R1 No. 1 and R2 No. 1)

● Hoist cable extends/retracts without crew input
HOIST ELECTRICAL RUNAWAY
HOIST ELECTRICAL RUNAWAY
- HOIST POWER - SECURE AT THE HOIST CONTROL PANEL
- ANNOUNCE - “HOIST RUNAWAY”
- ADVISE THE PF - “COMMITTED” OR “NOT COMMITTED”
If committed to the hoist or mission urgency dictates:
4. MANUAL OVERRIDE - ACTIVATE
If not committed to the hoist:
5. ABORT THE HOIST

● PF and/or FM unable to hear and/or talk during a hoist
LOST COMMUNICATIONS DURING HOISTING OPERATIONS
LOST COMMUNICATIONS DURING HOISTING OPERATIONS
- FM - TAP THE PF AND GIVE HOLD SIGNAL (closed fist)
- ADVISE THE PF - “COMMITTED” OR “NOT COMMITTED”
If committed:
3. COMMUNICATE BY VOCAL COMMANDS AND CONTINUE HOIST
If not committed:
4. ABORT THE HOIST
LOST RS (Day)
Corrective action - Day:
- MARK AND RECORD POSITION
- TRANSITION AND ESTABLISH RIGHT ORBIT
- LOOK AND LISTEN FOR RS SIGNAL
- WHEN VISUAL - RESUME HOVER POSITION
If these procedures fail to relocate RS:
5. COMPLETE LEAVING RS ONSCENE PROCEDURE
LOST RS (Night)
Corrective action - Night: 1. MARK AND RECORD POSITION 2. FLASH LANDING/HOVER LIGHT 3. LOOK AND LISTEN FOR RS SIGNAL 4. WHEN VISUAL - CEASE FLASHING LIGHT If these procedures fail to relocate RS: 5. COMPLETE LEAVING RS ONSCENE PROCEDURE
Signal: IN TROUBLE, NEED ASSISTANCE is observed.
EMERGENCY RECOVERY OF RS
EMERGENCY RECOVERY OF RS
- RESCUE BASKET - IF RIGGED AND EMPTY
- BARE HOIST HOOK - IF BASKET NOT READILY AVAILABLE
- WITH THE ASSISTANCE OF THE FM, THE RS SHALL ENTER THE CABIN BACKWARDS
LEAVING RS ONSCENE
- MARK AND RECORD POSITION
- DEPLOY LIGHTED RAFT
- DEPLOY DMB
