Important exam step in distal phalanx fractures?
-Two point discrimination (5mm is normal)
When to refer finger fractures?
-angulated or displaced unable to be reduced (soft tissue can get in the way making it difficult)
What is usual essential piece of material for management of these?
-U shaped padded aluminum splint
What is mallet finger?
-avulsion of extensor tendon leaving unopposed flexion of DIP
How to rule out mallet finger?
-must test active ROM at DIP joint
What films to order for distal phalanx fracture?
-AP, lateral, and oblique films
When to refer to ortho for mallet finger?
-only with volar subluxation, non-healing deformity, or swan neck deformity (hyperextension of PIP joint)
Splinting for mallet finger?
-splint just DIP joint in slight hyperextension CONTINUOUSLY for 6-8wks
What is jersey finger?
- flexor digitorum profundus avulsion (flexor tendon injury)
- type 1 = tendon retracts to palm (emergency)
