Gather and complete data, establish trust, teach the person about their health status and begin teaching health promotion and disease prevention.
What are traps for interviewing?
Proving false assurance or reassurance, Giving unwanted advice, using authority, distancing, using professional jargon, talking too much, interrupting, using “why” questions

What should written materials look like?
They should be at a level 5 grade level or below. They should be in bullet points, 12 pt font, appropriate pictures. Make sure to have the client teach back
What does a health history consist of?
Biographical data, source of history, reasons for seeking care, brief statement in persons OWN words and NOT a diagnosis, present health or history of present illness, past history, family history, review of symptoms, functional assessment.
When obtaining the patients symptoms find the
location, character of quality, quantity or severity, timing, aggravating or reliving factors, associated factors, client’s perceptions
What is the purpose of a review of symptoms?
To evaluate and present health state of each body system, another opportunity to obtain significant data about the patient’s health; obtain subjective data; usually done covering all symptoms and you need to accept or deny all symptoms.

dense organs; fluid filled; liver or cardiac tissue

no sounds; heard over muscle or tumors
diaphragm
used for high pitch sounds such as the heart, breast and bowl

used for soft low sounds such as a heart murmurs (hold lighter)
