20
Q

What does quality of deqi depend upon?

A
Constitution of the patient
Character of the disease
The part receiving the treatment
Season & weather
Needling technique
21
Q

What does deqi feel like to the practitioner

A

Tension & sinking or floating, like a fish taking bait.
Muscle jumps or squirms
Patient may smile or relax

22
Q

What functions does needle retention have?

A

Waits for qi arrival
regulates qi
supports even qi and peels evil qi
assists tonification and reducing

23
Q

Cautions for through/joined needling

A

Children, senility, pregnancy, schizophrenia
Careful not to injure internal organs/tissues
Depends upon differentiation rather than hunting for joined needling techniques

24
Q

What is joined needling used for

A

extending coverage of point’s indications, and strengthening needling interaction by linking eternal-internal related meridians, and facilitating qi arriving at affected part

25
Q

What 2 needling techniques are the basis of all other kinds of hand techniques

A

Lifting thrusting (push & pull), rotating (twisting)

26
Q

What are the three levels that the techniques of setting mountain fire and thorough cooling heaven operate through?

A

Heaven, person, earth

27
Q

With bufa (tonification), is the focus more on pushing or pulling, and is it with or against meridians?

A

Heavier push, milder pull, focus on forward rotating. Go along with meridians

28
Q

With xiefa (reducing), is the focus more on pushing or pulling, and is it with or against meridians?

A

heavier pull, milder push, focus on backward rotating. go against meridians

29
Q

With bufa, explain the breathing

A

Inhale by nose, exhale by mouth. Insert needle during exhalation. Withdraw needle during inhale

30
Q

With xiefa, explain breathing

A

exhale by nose, inhale by mouth. insert during inhalation. withdraw during exhalation

31
Q

With bufa, explain number of thrusts

A

Light ailments: 9 x 3 = 27 times, short pause after each 9
Severe: 7 x 7 = 49 times, short pause after each 7
Extreme: 9 x 9 times, pause after each 9

32
Q

With xiefa, explain number of thrusts

A

Light ailments: 6 x 3 = 18 times, pause after each 6
Severe 8 x 8 = 64 times, pause after each 8
Extreme 6 x 6 = 36 times, pause after each 6

33
Q

Compare speeds, direction of focus, and weight of thrust for bufa/xiefa

A

Bufa: slow heavier push, stress forward motion of thumb, quickly pull
Xiefa: quickly push, slow heavier pull, stress backwards rotation of thumb

34
Q

What is the four methods of transfer the qi overflying through meridians?

A

A. after perpendicular insert & qi arriving, pull needle from earth to heaven, then
B. put down the body of the needle with 2 or 3 fingers in order to change the direction of needling
C. needle toward each direction following order of upper, lower, left, and right
D. for each direction push needle slowly from shallow to deep (according three parts dividing), then pull directly to the heaven where you grab the pie

35
Q

What are the functions & indications of the 4 methods of xfer the qi overflying meridians?

A

A. dredge meridians & collaterals, boost meridian qi
B. tonify deficiency using xuji tonification (three in, one out)
C. brings qi from shallow to deep and spreads
D. various pain/bi syndromes

36
Q

Cautions for the 4 methods of xfer the qi overflying meridians?

A

A. can be applied either when qi is or isn’t grabbed
B. use over area of thicker muscle
C. Don’t use at chest, back, head, face, neck, large blood vessels or major nerves

37
Q

How do you perform setting mountain fire?

A

Pt does nasal inhale and mouth exhale. Insert into heaven during pt exhale, with nail press of left hand.
9 yang heavier push & forward stress/lighter pull (withdrawing 1-2/10th). Then do the same 9 in person part. Then same 9 in earth part. Pull the needle to heaven, pause, then withdraw slowly during inhale and press hole quickly & forcefully

38
Q

Clinical indications for setting mountain fire?

A

Chronic cold or yangqi xu such as declined wingmen fire or yuanqi xu of zangfu organs, meridians, collaterals.
Warm up yangqi and expel coldness to tx things like wind stroke, paralysis, cold-damp bi syndrome, cold extremities, cold ab pain, impotence, hernia, fifth-watch diarrhea, or external cold invasion by releasing the surface to create diaphoresis

39
Q

Cautions for setting mountain fire?

A

Use over thicker muscles, not end of four extremities/head/face.
Intensity depends on tolerance.
Stop manipulation during person or heaven part if they get warm or sweat.
Stop at the end of the full cycle even if there is no warmth/sweat. If warmth doesn’t come after 3 rounds of manipulation, retain the needle for 15 min, then warm will come.
Stimulation is longer than normal but duration shouldn’t be too long.
Pt should concentrate on tx.
Manipulation is better when qi is grabbed. Limit performance to the appropriate layer.
For sensitive pt, leave out breath/rotation factor.

40
Q

How do you perform thoroughly cooling heaven?

A

Patient mouth inhales and nasal heals. Insert needle to earth during inhale, using stretching technique of left hand.
Perform 6 yin number with heavier pull (light push) and backwards rotation. Then repeat at person layer, followed by heaven layer. Withdraw needle during exhale and enlarge needling hole and let it open.

41
Q

Indications for thoroughly cooling heaven

A

Excessive flaming of evil fire or ministerial fire.

Wind stroke, bi syndromes, dz caused by summer heat, high fever, delirium

42
Q

Cautions for thoroughly cooling heaven - are they the same as setting the mountain fire?

A

yes

43
Q

How do you perform red phoenix spreads its wings?

A

Insert needle to earth to grab qi. then pull to heaven and grab qi there. Thrust to person then perform push and pull while rotating. Release fingers after each rotation or each three rotations like a phoenix opening its wings. Perform right rotating and lifting during inhale, left rotating and thrusting during exhale.

44
Q

Indications for red phoenix spreads its wings

A

Boost & maintain qi sensation
Dredge meridians & collaterals
Reducing because xuji reducing (first deep & then shallow) is combined
Pain and bi syndromes

45
Q

Red phoenix spreads its wings cautions

A

Must grab qi at heaven first, then push needle.

Perform flying techniques at person level in a way of ‘wrap up the needle with the qi’

46
Q

Indications of tiger techniques

A

Promote qi mainly and used to reduce too: clear away heat & fire, expel wind & eliminate damp.
High fever and irritability, mental d/o, epilepsy, spasm, stiff neck caused by excess heat or fire or wind-phlegm

47
Q

How to you perform dragon techniques

A

Insert needle & grab the qi
Put tip of needle toward illness by put down the body of the needle during withdrawing to heaven part.
hold handle of needle, without pushing or pulling perform shaking from left to right or forward & backward within 90 degree like the rudder of a boat.
shake for 9 or 27 times. withdraw needle slowly & press hole quickly

48
Q

How to perform dragon and tiger fight alternatively

A

Method A: Insert needle to proper depth where qi arrives.
Perform dragon first (forward rotate 9 ties for yang tonificatin)
Then tiger (backward rotate 6 times for yin reducing)
Repeat

Method B: perform dragon wag its tail at heaven layer. Perform the techniques “tiger shake its head” at earth layer
