- Stick CFA at 45° angle
- Observe flashback
- Insert guidewire through needle
- Remove needle and leave guidewire in place
- Insert and advance sheath + dilator assembly OTW
- Remove dilator + wire and leave sheath

A
- Puncture location
(CFA below inguinal ligament and above SFA/Profunda bifurcation) - Vessel size
(4mm or greater unless 5-5-40 rule applies) - Deterrents
(Diffuse disease or focal within 2mm of puncture site, grafts or stents, pre-existing VAC, tortuous tissue track)

What is the 5-5-40 Rule?
A
If disease is present at the puncture site, AngioSeal can be safely deployed if:
No disease is present within 5mm of access site
Vessel is 5mm or greater
Luminal narrowing is <40%
