A
  1. Locate CFA via Ultrasound
  2. Stick CFA at 45° angle
  3. Observe flashback
  4. Insert guidewire through needle
  5. Remove needle and leave guidewire in place
  6. Insert and advance sheath + dilator assembly OTW
  7. Remove dilator + wire and leave sheath

    (CFA below inguinal ligament and above SFA/Profunda bifurcation)
  2. Vessel size
    (4mm or greater unless 5-5-40 rule applies)
  3. Deterrents
    (Diffuse disease or focal within 2mm of puncture site, grafts or stents, pre-existing VAC, tortuous tissue track)

What is the 5-5-40 Rule?

A

If disease is present at the puncture site, AngioSeal can be safely deployed if:
No disease is present within 5mm of access site
Vessel is 5mm or greater
Luminal narrowing is <40%
