A disorder of central pain processing characterised by chronic widespread pain in all 4 quadrants of the body

Pathogenesis of fibromyalgia
A
- It can induced by deliberate sleep deprivation > sleep disturbance is the most common trigger
- reduced REM sleep + delta wave sleep in EEG
- causes hyper activation in response to noxious stimulation + neural activation in brain regions associated with pain perception

Symptoms + signs of fibromyalgia
A
- joint/muscle stiffness
- profound fatigue
- unrefreshed sleep
- numbness
- headaches
- IBS
- depression + anxiety
- poor concentration
- no physical abnormalities to examination to MSK or neurological systems

Management of fibromyalgia
A
- measures to improve sleep + physical activity levels
- low dose amitrypytline
- pregabalin
- cognitive behavioural therapy
- avoid opiates
