List some medications used in the management of symptomatic fibroids
For menorrhagia:
NSAIDS, Tranexamic acid
OCP, Depo provera, GnRH agonist
List some differential diagnoses for deep dyspareunia
Uterine fibroids, endometriosis, adenomyosis, PID, tubo-ovarian abscess, ovarian cyst
What are some of the most common clinical presentations of fibroids?
- dysmenorrhoea
- infertility
- menorrhagia
- acute pelvic pain (torsion)
- pressure/bulk symptoms (increased abdominal girth, urinary frequency + urgency, constipation)
Which type of fibroid is most likely to be symptomatic?
Submucosal
Fibroids may complicate a pregnancy, how?
Recurrent pregnancy loss
Pain during prgnancy secondary to growth +/- necrosis
Failure of the presenting part to engage (if in lower segment)
Obstructed labour
Malpresentation
Difficult entry at C/S - occasionally necessitating myomectomy with concurrent severe haemorrhage
Post partum haemorrhage
What is the most common demographic for uterine fibroids?
Pre-menopausal over the age of 35 years
What findings would you expect of a lady with fibroids on bimanual examination
Firm, enlarged and often irregular shaped uterus
Which hormones are implicated in the pathogenesis of fibroids?
oestrogen & progesterone
