What is a fibroadenoma?
Common benign breast lump in pre-menopausal women
Due to focal growth of stromal + epithelial components
What is a fibroadenoma aka?
Breast mouse
Describe 4 features of a fibroadenoma
Rubbery firm mobile mass
Solitary, well circumscribed + smooth
Non-tender
Typically 1-2cm diameter
What size is a fibroadenoma classified as a ‘giant fibroadenoma’?
> 5cm
Give 2 epidemiological facts on fibroadenoma
Common in young (peak 20-24y)
Most common benign breast lesion in <35s
What causes fibroadenomas?
Idiopathic
Possible hormonal aetiology: present/ change with menstruation/ pregnancy
Describe changes in fibroadenomas during pregnancy and menopause
Growth stimulated by increased oestrogen during pregnancy
Revert after menopause
What investigations may be performed in patients with fibroadenoma?
Triple assessment
USS (<30)
Mammogram (>30)
Biopsy if >5cm/ ?malignancy
Describe the progression of fibroadenomas
50% resolve spontaneously
25% stay the same
25% increase in size
What are the management approaches for fibroadenoma?
Conservative: reassurance + routine clinical breast exams
Surgical: excisional biopsy
What are the 3 main indications for removal of fibroadenoma?
Size >3cm/ rapid growth
Symptomatic/ cosmetic concerns
Suspected malignancy: cellular on histology (risk missing phyllodes)
What is the prognosis for patients with fibroadenoma?
Good
No increased risk of carcinoma
