GI: Gastroenteritis - What is it?
bacterial/viral infection causes osmotic diarrhea
GI: Gastroenteritis - Predisposing Conditions
ill contacts, daycare, other (raw food, reptiles)
GI: Gastroenteritis - History
diarrhea, nausea/vomiting, +/- fever, +/- abdominal pain, symptoms of dehydration
GI: Gastroenteritis - Physical Exam
well or ill appearing, normal vitals except fever, signs of dehydration (decreased skin turgor, dry mucous membranes, tachycardia, sunken eyes, listlessness, +/- abdominal tenderness
GI: IBD - What is it?
Chronic inflammation of the intestinal tract. Likely related to dysregulation of mucosal immune function. (Chron’s Disease: mouth to anus, Ulcerative Colitis: primary colon and rectum)
GI: IBD - Predisposing Conditions
family history of Chron’s or Ulcerative Colitis, adolescents/young adults (15-30), elderly (60-80), certain genetic syndromes
GI: IBD - History
usually chronic (may be acute), diarrhea & abdominal pain, (Ulcerative colitis: rectal bleeding, tenesmus, fever, anorexia, weight loss), (Crohn’s: RLQ ileocolitis or mass, fever, weight loss, perianal pain, incontinence, extra- GI symptoms (arthritis, depression))
GI: IBD - Physical Exam
+/- fever & tachycardia, weight loss, abdominal tenderness (RLQ mass = Crohn’s), perirectal abscess/perianal fistula (Crohn’s), Tender rectal exam (both), +/- extra-GI signs (Crohn’s)
Chronic Infections - Predisposing Conditions
some type of exposure: ill contacts, sexual contacts, drug use, other exposures
Chronic Infections - History
constitutional: fever, weight loss, night sweats, lymph node enlargement, fatigue, symptoms of anemia (dyspnea on exertion)
Chronic Infections - Physical Exam
may be ill-appearing overall, +/- fever & tachycardia, lymphadenopathy, skin rash or jaundice, +/- pallor, organomegaly
Malignancy - Predisposing Conditions
prior history of cancer, family history, recent infection, different exposures
Malignancy - History
acute or chronic symptoms, commonly associated with fever, likely progressive or sometimes gradual, weight loss, lymphadenopathy, jaundice
Malignancy - Physical Exam
tachycardia, +/- tachypnea, +/- fever, signs of anemia (pallor, heart murmur), +/- lymphadenopathy/organomegaly/ill apperance
Autoimmune Disease - What is it?
inflammation, immune complex mediated
Autoimmune Disease - Predisposing Conditions
Family history of autoimmune disease, more common in women
Autoimmune Disease - History
often associated with fever (acute or chronic), may be associated with pain, associated symptoms: rash,arthritis,myalgia,pallor,neurologic symptoms,respiratory symptoms,fever,weight loss
Autoimmune Disease - Physical Exam
+/- fever, rash, lymphadenopathy, arthritis, tenderness over specific points
