how to treat nursemaid’s elbow
closed reduction with foreearm supination and elbow flexion:
immobilize the elbow and palpate the region of hte radial head
apply axial compmression at the wrist while supinating the forearm and flexing the elbow
still technique for posterior radial head
maintain posterior pressure on the radial head, place the forearm in pronation, add compression, take forearm into supination
test for malingering?
Hoover sign
put hand on good leg, ask them to lift bad leg. Should feel good leg make an attempt.
left short leg. Early finding?
tenderness over left iliolumbar ligament
scoliotic curve sidebent left and rotated right –> What with scapula?
inferior left inferior angle of scapula
right short leg –> ? pelvis? and other stuff?
pelvic side shift left
Group curve sidebends away from the short leg
long leg is internally rotated and the foot is pronated
Innominate is anterior on the short side
Compensatory S schaped scoliosis
chornic posutral compensation for a short limb
most inferior verteebrae in the group curve remain sidebent away from teh short limb
2ndary compensatory curve develops more superiorly that sidebends the opposite way
strained adductors. fascial lines–> what else?
longus coli and the tib/fib interosseous membrane
muscle imbalance
disturbance of function
afferent activity to cord, brainstem, cortex
efferent pathway to alpha motor neuron–> contraction
muscle spindles adapt–> ongoing aberrant muscle tone
important aspects of correcting muscle imbalance?
- proprioceptive balance
- stretch facilitated muscles
- re-educate/ strengthen the weak ones (teach correct muscle firing patterns)
craniosacral: index finger on right more superior than left, pinky is inferior to left. dx?
Right torsion
cranio: oth index fingers go superior, both pinkies go inferior. dx?
inferior vertical shear
whichever way your wrists go is the shear
