Worked O.T., stopped 2Xs excess of 15 mins each

RS thought you were acting erratically; made mandatory EAP referral (11 am on 11/3/16). Left at 1:30 pm but did no orders that day.

approx 1:10 pm notified Christalinas that truck was stolen; Super Lombard called back and said FB told her she left keys on the seat;; Magnetic key chain disconnected and keys stuck to passenger-side door; MG explains difference in statement from Barbara Lombard, Luis Christalinas, Andwelle Williams

FB aware that carrying non-DWP issued metallic objects, while working on energized services, is a safety hazard?
Yes
Service Order worked on supposed to be barrel-locked

11 am meeting with Personnel; RS says that FB had personnel call him when appointment was over which was 12:30 pm. Records show she did not do first order until 3:30 pm

12:28 pm Dispatch notified me that you had not completed your first order, and had already spent 2 hours 11 minutes idle.

Went to location at Mall at 1:10 pm with 99 Cent store. Asked why not working, said took too long for lunch.
Approximately 6:15 pm, when returned to office, asked to write statement of your activities from 8:30 am to 1:30 pm (paid O.T. to write)

Next morning looked at statement and no details on what were doing.

Called meeting. Attendees: Richard Strauss, Danny Jaramillo (Union Steward), Mark Gentili
Asked why no cones, showed Danny the photograph showing no cones outside 99 cent store-FB said she did not put cones there. Told her it is mandatory to put cones. Told her she wrote report covering day before 3/1/17 with a tiny part for 3/2/17.

Cone discussion was mandatory.. ked for 2nd statement, said she is not good writer, decided Danny could help her. 10:15 AM concluded meeting
