Describe Blind Toggle Pin Abomasopexy in left displaced abomasum correction
Only used in USA
Cast onto back and insert two toggles via a cannula pushed through ventral body wall
Test fluid with pH strip to confirm siting
Describe right flank ometopexy in correcting left displaced abomasum
Enter right flank through 15cm incision one hands breadth behind last rib
Locate abomasum by passing left hand toward pelvis
Deflate by pressing over organ ventrally and caudally
Reposition by sliding arm under rumen and lift it while pushing palm downward bringing organ towards body
Locate omental fat and trace cranially and ventrally and continue to locate knobbly pylorus
Suture fat adjacent to this into abdominal wall using cat gut
Lock fat in using mattress suture through musculature and through fat
Close transversalis and peritoneum with continuous suture
Close oblique muscles and then close skin
Describe a left flank omentopexy in correcting left displaced abomasum
Enter left flank through 15cm incision caudal to last rib
Pass a long nylon suture through fat several times leaving two, one metre long ends
Pass through ventral body wall 10cm apart
Tighten the sutures and tie externally with 10 throws on the knot
Cut suture flush with skin after 3 weeks
Describe right paramedian abomaspexy in correcting left displaced abomasum
Sedate cow and cast onto back
Incise to right of the midline and locate abomasum, reposition and suture to wall
Describe the post operative care after LDA corrections
Don’t put back onto full concentrate rations
Fill rumen with hay or silage for at least 4 days then slowly introduce the concentrate
Describe the process of correcting a right displaced abomasum using right flank correction and omentopexy
Right flank laparotomy incision locating abomasum in right abdomen and identify twist
Place a running suture around greater curvature then another around a 2cm area
Insert a stomach tube - drain off fluid and gas before closing
Correct displacement
Volvulus must be corrected first by rotating organ clockwise around a vertical axis then flexion should be corrected
Then locate pylorus and perform an omentopexy
What fluid therapy should be provided with a right displaced abomasum?
Isotonic saline - 30l IV via a pump
Continue with oral fluids 20-40l with 100g KCl BID
What is the post-op care for right displaced abomasum?
Slower to recover than LDA so keep on silage or hay only for 7 days before introducing concentrate
What needs to be administered before surgical treatment of caecal dilatation and volvulus?
IV fluids with Flunixin Meglumnine
Describe a right flank laparotomy when correcting caecal dilatation and volvulus
Deliver caecum apex through incision
Pack abdomen and perform typhlotomy (caecal incision) at apex
Empty contents
If devitalised, apply bowel clamps and resect necrotic tissue
Repair with double row of continuous inverting sutures
