Making healthcare decisions and understanding treatments.

Performing essential activities to implement a regimine. - family affects adherence rates.
How does health affect romantic partners.
It affects both partners (shared experience and stress)
Partners are the 1st people we turn to for social support.

How do couples cope with chronic disease? Types of this?
Evidence is based on self reports BUT we need to take perspective of both partners!
DYADIC APPRAISAL AND DYADIC COPING
Cycle of Patient coping/appraisal and adjustment and Spousal Coping/appraisal/adjustment
Dyadic appraisal:
- illness representation
- illness ownership
- shared stressor
Dyadic coping: 4 types, uninvolved, supportive (emotional and tangible), collaborative (solve problems) and control (dominates action).
What is diabetes?
Insulin hormone that regulates blood sugar
type 1: insulin dependant, develops in childhood. Not producing enought insulin, immune system attacks insulin producing beta cells of the pancreas.
type 2: common, related to obesity, high blood sugar, insulin resistant, need to manage diet.
Describe managing diabetes?
Glucose control
Complex behavioral changes
Stressful
Nonadherence can lead to acute chronic health problems.
What are the 3 ways spouses get involved in diabetes management?
Support: showed appreciation, being good spouse
Persuasion: convince person to stick to their diet or motivate them
Pressure: criticize patient’s choices
Contributions are not always helpful, pressure is really bad.
How do spouses impact diabetes management?
Studies have shown that support is happening more, persuasion is variable and not many poeple are getting pressured.
Support gives better dietary adherence and pressure gives a lot of distress.
What is cancer?
Diseases characterized by abnormal cell growth. Leading cause of death in Canada.
- Health behaviors are known to be related to some cancer risks
- psychosocial factors such as stress and depression influence course of cancer but don’t CAUSE it (etiology)
How do couples with a partner having cancer cope?
effect each other. if one has a bad day, both will (if they are collaberating well) if they dont collaberate well, one partner’s bad day doen’st affect the others.
being a support provider makes you vulnerable.
What is arthritis?
Diseases involving joins and inflammation of them. common disease.
RA (rhumatoid) is inflammation of hands and smaller joints. really painful and correlated with depression. need for social support. Uses rheumatoid arthritis disease activity index.
How does spousal depression predict future disease activity?
Disease, depression and spousal depression are all indicative of patients disease activity one year later!!!
Spouse’s depression predict disease one year later.

Depressed spouses are less liekly to provide satisfactory support and more likley to have maladaptive coping strategies.
