Recurrent Unilateral Facial Nerve Paralysis - Differential Diagnosis
I. Infectious:
1) Ramsay-Hunt Syndrome
2) Herpes Simplex Virus
3) HIV
4) Lyme disease
II. Inflammatory:
1) Sarcoidosis
2) Sjogren’s syndrome
3) GPA/Wegener’s Granulomatosis
4) Systemic lupus erythematosus
III. Neoplastic:
1) Schwannoma
2) Parotid gland neoplasms
3) Leukemia/Lymphoma
IV. Structural:
1) Traumatic neuroma
V. Other:
1) Melkersson-Rosenthal Syndrome
House-Brackmann Facial Paralysis Scale
I - Normal
II - Mild dysfunction (slight weakness, normal symmetry at rest)
III - Moderate dysfunction (obvious but not disfiguring weakness with synkinesis, normal symmetry at rest)
- Complete eye closure with maximal effort, good forehead movement
IV - Moderately severe dysfunction (obvious and disfiguring asymmetry, significant synkinesis) Incomplete eye closure, moderate forehead movement
V - Severe dysfunction (barely perceptible motion)
VI - Total paralysis (no movement)
