Extrinsic Allergic Alveolitis Flashcards
what is extrinsic allergic alveolitis (EAA) or hypersensitivity pneumonitis?
condition caused by hypersensitivity induced lung damage due to a variety of inhaled organic particles
what is extrinsic allergic alveolitis caused by?
- immune-complex mediated tissue damage (type III)
- delayed hypersensitivity (type IV) is thought to play a role in EAA, especially in the chronic phase
name some examples of extrinsic allergic alveolitis
- bird fanciers lung
- farmers lung
- malt workers lung
- mushroom workers lung
what is bird fanciers lung caused by?
avian proteins from bird droppings
what is farmers lung caused by?
spores of saccharopolyspora rectivirgula from wet hay
what is malt workers lung caused by?
aspergillus clavatus
what is mushroom workers lung caused by?
thermophilic actinomycetes
how does acute extrinsic allergic alveolitis present?
- dyspnoea
- dry cough
- fever
when does acute extrinsic allergic alveolitis present?
4-8 hours after exposure
how does chronic extrinsic allergic alveolitis present?
- lethargy
- dyspnoea
- productive cough
- anorexia and weight loss
when does chronic extrinsic allergic alveolitis present?
occurs weeks-months after exposure
what x-ray findings are seen in extrinsic allergic alveolitis?
upper/mid-zone fibrosis
what is the management of extrinsic allergic alveolitis?
- avoid precipitating factors
- oral glucocorticoids
