Extremity Counterstrain Flashcards
List the counterstrain steps
- Find most signficiant tenderpoint
- Physician establishes a tenderness scale
- Monitor Tenderpoint throughout
- Place patient in “Position of Ease” of at least 70% improvement
- Hold for 90 seconds
- Slowly return to neutral
- Recheck tenderness and associated TART findings
Supraspinatus counterstrain
Location?
Midsupraspinatus muscle just superior to spine of scapulae
Supraspinatus counterstrain
Pt. position
Doc position
Pt: Supine
Doc: Ipsilateral to tenderpoint
Supraspinatus counterstrain
Treatment Positioning
F Abd ER
- Patient’s arm is flexed 45 degrees, abducted 45 degrees, and externally rotated*
- Think Statue of Liberty*
Levator Scapulae
Location
In levator at the superior angle of the scapula
Levator Scapulae
Pt position
Doc position
Pt: Prone, head rotated away
Doc: Ipsilateral to TP
Levator Scapulae
Treatment positioning
IR Abd traction
IR pt’s shoulder, add mild-mod traction with minimal abduction
Subscapularis
Location
Anterolateral border of the scapula on the subscapularis muscle (press posterior and medially)
Subscapularis
Pt & Doc position?
Pt: Supine
Doc: Seated ipsilateral
Subscapularis
Treatment Positioning
E IR
Patient’s shoulder extended and internally rotated, traction can help
Biceps Brachii (Long Head)
Location
Over long head of biceps tendon in bicipital groove
Biceps Brachii (Long Head)
Pt & Doc Position
Pt: Supine
Doc: Ipsilateral
Biceps Brachii (Long Head)
Treatment positioning
F Abd ir
Elbow & shoulder flexed, arm is minimally abducted and internally rotated
Dr. Joy’s way of remebering: place hand on head (F Abd ir) and think “ugh this is the LONGEST technique”
Biceps Brachii (Short Head) & Coracobrachialis
Location
Inferolateral aspect of the coracoid process on short head tendon of biceps or coracobrachialis
Biceps Brachii (Short Head) & Coracobrachialis
Pt & Doc position
Pt: Supine
Doc: Ipsilateral
Biceps Brachii (Short Head) & Coracobrachialis
Treatment positioning
F Add ir
Elbow and shoulder flexed, arm is minimally adducted and internally rotated
Note: like long head except ABd –> ADd
Radial Head - Lateral (Supinator)
Location
Anterolateral aspect of the radial head at supinator attachment
Radial Head - Lateral (Supinator)
Pt & Doc position
Pt: Supine or seated
Doc: ipsilateral
Radial Head - Lateral (Supinator)
Treatment positioning
E SUP Val
Pt’s elbow in full Extension, forearm markedly supinated, fine tune with vaLgus force
Medial Epicondyle (Pronator Teres)
Location
Near medial epicondyle of humerus associated with common flexor tendon and pronator teres
Medial Epicondyle (Pronator Teres)
Pt & Doc Position
Pt: Supine or seated

Medial Epicondyle (Pronator Teres)
Treatment positioning
F PRO Add
Pt’s elbow Flexed, marked pronation, forearm slightly aDducted
Dorsal Wrist (Extensor Carpi Radialis)
