MRC 5/5 power grading
Movement against gravity with full power against resistance
MRC 4/5 power grading
Movement against gravity with reduced power against resistance
4-,4 and 4+ indicate slight moderate and strong resistance respectively
MRC 3/5 motor power
Movement against gravity only without applied resistance
MRC 2/5
Muscle contraction with active movement only when gravity is eliminated
MRC 1/5
Flicker of muscle contraction seen, no movement
MRC 0/5
No muscle contraction
What is hemiplegic gait
Px lurches his upper body toward the un paralysed side to elevate the pelvis and swing the paralysed leg around. Plantar flexed foot scrapes along the ground.
What is Apraxic gait
Slow and shuffling. Stride length smaller. Can lose balance when turning.
In Parkinson’s there is a loss of arm swinging with rapid steps
What is steppage gait
Paralysis of the dorsifkexors results in drop foot. Patient has to lift knee high and slap sound when foot hits the floor.
Unilateral - nerve palsy
Bilateral - polyneuropathy
What is ataxic gait
Wide based gait. Patient sways while walking. Heel to toe make more pronounced.
To complete a spine exam
Full upper and lower neuro
Examine peripheral pulses in upper and lower limbs
Neck pain - examine the shoulder
Lower back pain - examine hips and abdo exam
