A

NOTHING with involved side. BL Scap squeezes. Uninolved side adduction. A lot of manual.

Purpose of performing PROM on patients who have had surgery?

A

SO that the joint does not become stiff from adhesion/fibrosis/arthrofibrosis/fibrotic adhesions, does not lose the ROM which will be what we are trying to get back to functional means, and AROM may damage the integrity of the surgery site.

OA/RA reasons for treatment?

A

OA/RA avoid movement usually due to pain, or can not reach the range due to inflammation of the joint. When someone avoids movement… decreases ROM, mechanoreceptors and muscle spinde become diminished with decreased/altered sensory information which can impair balance and proprioception/kinestetic awareness.

Will you apply heavy resistance or stretching to patient who have joint inflammation?

A

No. Do not apply heavy resistance. Especially to those with OP. Apply gentle isometrics > ROM. Keep it low intensity with high repetitions. pain free arc’s and multiple angle isometrics.

Mobilizations for those with inflammatory diseases?

A

yes. grade 1 and 2 for pain/increase synovial fluid in a hypomobile joint.

contraindications for those with these diseases?

A

VIGOROUS stretching/ manipulations due to joint capsule ligaments and tendons that may be stucturally weakened by RA/steroids. Be cautious with the grade 1 and 2 mobilizations.

OP patients? Things to think about?

A

With OP one wants weight bearing exercises such as walking jogging, climbing stairs, jumping, leg arm bike. Also they usually present kyphosis due to vertebrae becoming more wedge shape, avoid trunk flexion exercises, and flexion with rotation exercises to decrease risk of vertebral compression fx.

What is the self perpetuating cycle of muscle spasm?

A

a whole lot of things make > restricted movement > tissue ishemia > pain > muscle spasm > more restricted movement… etc
