Weight >150: ceftriaxone 1 gm x1

Gentamicin 240mg + azithromycin 2mg
Cefepime 800mg

Primary and secondary syphilis
Benzathine penicillin G 2.4m units IM 1 dose
PCN allergy: doxycycline 100mg 14 days, tetracycline 500 mg 14 days, azithromycin 2g dose
Early latent syphilis: <1 year duration
Benzathine penicillin G 2.4m units IM 1 dose
PCN allergy: doxycycline 100mg 14 days, tetracycline 500 mg 14 days
Late latent/ tertiary syphilis: >1 year duration
Benzathine penicillin G 2.4m units IM once weekly for 3 weeks
PCN allergy: doxycycline 100mg 28 days, tetracycline 500 mg 28 day
Neurosyphilis
Aqueous crystalline penicillin G 3-4m units q4h for 10-14 days
May add Benzathine penicillin G 2.4m units IM once weekly for 3 weeks
Procaine penicillin 2.4 units IM + probenecid 500 mg QID for 10-14 days
PCN allergy: ceftriaxone 2 g IM or IV for 10x14 days
Syphilis treatment in pregnancy
penicillin is the only agent that protects the fetus
Chlamydia treatment - Adolescents and adults
Preferred: Doxycycline 100mg x 7 days
Alternative regimens:
azithromycin 1 gram x 1 dose
levofloxacin 500mg x 7 days
Chlamydia treatment - pregnancy
preferred: azithromycin 500mg x 1 dose
alternative: amoxicillin 500mg x 7 days
Mycoplasma genitalium treatment - macrolide-susceptibile
doxycycline 100 x 7 days
then azithromycin 1gram 1 dose
then azithromycin 500mg for 3 doses
Mycoplasma genitalium treatment - macrolide-resistant or testing not available
doxycycline 100 x 7 days
then moxifloxacin 400mg x 7 days
Genital Herpes treatment - initial
first clinical episode (treat 7-10 days)
acyclovir 400 TID
famciclovir 250 TID
valacyclovir 1g BID
Genital Herpes treatment - reccurent
acyclovir 800 BID 5 days or acyclovir 800 TID 2 days
famciclovir 125mg BID 5 days or famciclovir 1g BID 1 days
valacyclovir 500mg BID 3 days or valacyclovir 1g 5 days
Genital Herpes treatment - additional treatment (severe)
acyclovir 5-10 mg/kg/dose IV q8h 2-7 days
Genital Herpes treatment - suppressive
acyclovir 400 BID
famciclovir 250 BID
valacyclovir 500/1000 QD
Trichomoniasis treatment
men: metronidazole 2 gram
women and HIV+: metronidazole 500mg BID x 7days
pelvic inflamamtory disease
Ceftriaxone + doxycyline + metronidazole
alternative: unasyn + doxycycline
