Distinguish between Medical Nutrition Therapy (MNT) and the Nutrition Care Process
Medical Nutrition Therapy (MNT): Medical Nutrition Therapy is Nutritional care provided to a client with a medical diagnosis.
Nutrition Care Process: a system problem-solving method that dietetic professionals use to critically think and make decisions to address a nutrition related problem and provide effective nutrition care.
Describe the standardized nutrition care process (NCP) and model
The standardized Nutrition care is to have a foundation to gather valid and reliable data on how quality nutrition care provided by a professional improves the quality of health care provided.
The nutrition care process model is the relationship between the patient and the RD. There are 3 rings and one thing feeding it and the outcomes.
The interior ring consists of four steps
S1: nutrition assessment;(Anthropometrics, Biochemical assessment, Client history, diet history, nutrition related physical findings)
S2: Nutritional Dx; for nutrition dx we identify the most important/primary/high risk nutrition problem. find patterns in the data. find the etiology/cause of the nutrition related problem.
S3: Nutritional intervention; we implement nutritional care by planning/strategizing, prioritizing Nutritional Dx, give resources to the client
S4: Monitor and evaluate; monitoring progress (A,B,C,DP) and document progress. The purpose to monitoring and evaluating is to determine progress towards the goal.
Second ring (middle ring): list the intrinsic factors affecting NCP.
- Nutrition and dietetic knowledge, collaboration, skills and competence, communication, evidence-based practice, critical thinking, documentation, code of ethics.
The third ring (outer ring): list the concepts that define the environment in which we provide nutrition care.
- Healthcare system, practice settings, social systems, economics
What feeds the NCP model
- screening and referrals(re-assessment): identify risk factors, by using tools/methods
What comes out the NCP model
- outcomes management system: research, aggregated data, conduct continuos quality improvement.
How to determine the nutrition risk of a patient
by screening and using tools such as questionnaires, observations and standards
