mechanical ventilation (>48 hours), coagulopathy, drugs that increase bleed risk (warfarin, heparin, steroids), shock/sepsis/hypotension/vasopressors, hepatic/renal failure, multiple trauma, burns >35% BSA, organ transplant, head/spinal trauma, history of upper GI bleed/PUD
