Q
- What ABI contraindicates the use of compression therapy?
A
- ABI can be used to determine how much compression can be safely applied without compromising arterial blood flow. Normal ABI = 1-1.3. ABI less than 0.9 = arterial disease. ABI 0.6 -0.8 = borderline ischemia. ABI less than 0.5 = severe ischemia. ABI between 0.8 -1.0 indicates venous etiology, however ABI 0.5-0.8 indicates mixed venous and arterial etiology. Compression should not be applied if ABI is less than 0.5, as this is indicative of severe arterial disease. If using high or moderate compression (50 – 30 mmHg), ABI should be above 0.8; Low or moderate compression (25-35 mm Hg), ABI should be above 0.5
- In a study of 18 diabetic patients with edema and with mean ABI 1.10, Wu et al found that mild compression therapy could be applied via diabetic socks without complication and with significant reduction in calf and foot circumference.
